Overview

This application is used by authorized 4WEB Medical sales representatives to create and submit implant sales orders from the field.

Information Collected

• User details (name, email, organization, distributor)
• Order details (hospital, surgeon, surgery date, purchase order number, items)
• A patient identifier entered by the user to associate the order with a surgical case
• An optional photograph of the hospital patient label, where captured by the user
• Photographs of parts, trays and kits
• Witness signatures captured for order sign-off
• Basic system activity (sign-ins, order submissions, record access, and synchronization events)

The application does not collect diagnoses, imaging, prescriptions or clinical records.

Camera and Photo Access

The application requests camera and photo library access solely to attach order documentation. Photographs are used only for the order they are attached to. The application does not access photographs other than those the user selects.

Biometric Unlock

Where enabled, Face ID or fingerprint unlock is handled entirely by the device operating system. Biometric data never leaves the device and is never transmitted to, or accessible by, 4WEB Medical.

Information Stored on Your Device

To allow order entry without a network connection, the application stores order data, reference data and attached photographs locally on the device, including the patient identifier and any patient label photograph. This data remains on the device until the order is submitted and synchronized.

Application lock, together with the device passcode or biometric protection, safeguards this stored information. Users must keep device security enabled and report lost or stolen devices promptly.

• Meet legal and regulatory requirements

Data Protection

Information is handled in accordance with applicable healthcare privacy laws, including the Health Insurance Portability and Accountability Act. Safeguards include:

• Encrypted transmission
• Secure storage
• Controlled access by assigned role
• Activity logging of sign-ins, record access and administrative actions

Data Sharing

Information is shared only:

• With authorized healthcare providers or facilities
• With approved service providers under required agreements
• When required by law

Submitted orders are transmitted to 4WEB Medical’s order management system for fulfillment.

Data Retention

Information is retained as needed for operational and regulatory purposes.

User Responsibility

Users must:

• Be authorized to submit the information
• Enter only necessary and accurate data
• Protect their account credentials
• Maintain device security, including a passcode or biometric lock

Authorization

By using this application, you confirm that:

• You are authorized to submit the information
• The information is accurate to your knowledge
• Your use complies with applicable laws

Account Use

You are responsible for:

• Maintaining the security of your login
• Using only your assigned account
• Not sharing access with others

Device Use

The application stores order information on the device, so the device itself forms part of the safeguards. You are responsible for:

• Keeping a passcode or biometric lock enabled on the device
• Keeping the application lock enabled
• Reporting a lost or stolen device promptly so access can be revoked
• Not retaining or forwarding order information outside the application

Acceptable Use

You agree not to:

• Access data without authorization
• Upload unrelated or unlawful content
• Interfere with application functionality or security

Data & Security

Reasonable safeguards are implemented to protect submitted information. Users are responsible for employing secure handling practices when capturing and transferring data.

Availability

The application may be updated, modified, or temporarily unavailable as needed and without notification.

Termination

Access may be suspended or revoked for misuse or security concerns.

Governing Law

These Terms are governed by the laws of the State of Texas. Any disputes arising from or related to use of this application shall be resolved in the state or federal courts located in Texas.
